All forms listed below are available electronically and should be completed on the Genesis Parent Portal. If you are unable to access Genesis, please contact 908.526.8900 ext. 7361. Access to student schedules will not available until all required forms are completed. Please note: these forms are required to attend SCVTHS.

• Complete the Health Office Emergency Form on the parent portal (annual form)
• Complete and acknowledge Student Information Verification form in the parent portal (annual form)
• Review and submit the Weapons Awareness Agreement form on the parent portal
• Review publicity preferences and submit the Publicity Release on the parent portal
• Review and submit the Eye Protection Areas & Devices form on the parent portal
• Review and submit the Acceptable Use Of Technology Agreement form on the parent portal
